About Our SMS Program
Cornerstone Therapy and Wellness (Cornerstone Counseling and Wellness LLC) operates an SMS messaging program to communicate with clients of our outpatient mental health practice. These SMS Terms of Service govern your participation in our text messaging program. By opting in, you agree to these terms.
Program Name: Cornerstone Therapy and Wellness SMS Notifications
Program Description: Appointment reminders, billing notifications, scheduling updates, intake information, and administrative messages related to your care at Cornerstone Therapy and Wellness.
How to Opt In
Clients opt in to SMS communications by completing our new patient registration form in our secure, HIPAA-compliant client portal at https://cornerstonetherapy.insynchcs.com/. The portal is accessed after a prospective client requests services through our scheduling page at https://cornerstonetherapy.com/get-scheduled-today/.
The registration form includes a separate, unchecked SMS consent checkbox that the client must affirmatively select to opt in. The checkbox is not bundled with any other consent and is not required to receive clinical services.
Consent is captured separately from all other consents. It is not bundled with HIPAA acknowledgments, financial agreements, or treatment consents. You can decline SMS and still receive full clinical services from Cornerstone Therapy and Wellness.
Consent language on the registration form reads:
"I consent to receive SMS text messages from Cornerstone Therapy and Wellness, including appointment reminders, scheduling confirmations, intake form links, billing notifications, and administrative messages. Message frequency varies. Message and data rates may apply. I can reply STOP to opt out or HELP for help. My mobile number will not be shared with third parties or affiliates for marketing or promotional purposes."

Message Frequency
Message frequency varies based on your appointment schedule, billing activity, and administrative needs. You may receive multiple messages per week depending on your level of engagement with our practice. We will not send unsolicited marketing messages.
Message and Data Rates
Message and data rates may apply. Text messages sent and received may be subject to charges from your mobile carrier based on your plan. Cornerstone Therapy and Wellness does not charge a fee for participation in our SMS program. Please contact your mobile carrier for details about your messaging plan.
How to Opt Out
You may opt out of our SMS program at any time by replying STOP to any message you receive from us. After opting out, you will receive a single confirmation message and no further SMS messages will be sent. To re-enroll, text START from the same mobile number, or contact our office directly.

Privacy and Mobile Number Sharing
Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes.
All categories of mobile information collected through this SMS program, including phone numbers and consent records, are restricted from being shared with any third parties for marketing or promotional purposes.
The only third party that receives your mobile number is Twilio, our SMS messaging platform provider, for the sole purpose of delivering the messages you have consented to receive. We may also disclose information as required by law or permitted under HIPAA.

HIPAA and Protected Health Information
- We will never include protected health information (PHI), diagnosis details, or clinical content in SMS messages beyond the minimum necessary for the message purpose
- We do not sell or share your phone number with third parties for marketing or promotional purposes
- SMS messages are not a secure method of communication for sensitive clinical matters. Please call our office for any clinical questions
- Standard SMS is not an encrypted communication channel. Messages may be visible to anyone with access to your mobile device
Supported Carriers
Our SMS program is supported by most major United States wireless carriers, including AT&T, Verizon, T-Mobile, US Cellular, and most regional carriers. Carrier support may vary. We are not responsible for delays or failures in SMS delivery caused by your mobile carrier.
Eligibility
Our SMS program is available to clients who are 18 years of age or older, or to the parent or legal guardian of a minor client who has authorized SMS communications on the minor's behalf.
Changes to These Terms
We may update these SMS Terms of Service at any time. The date at the top of this page reflects the most recent revision. Continued participation in our SMS program following any update constitutes your acceptance of the revised terms.
